Learn how to complete and track child medication forms easily!

Medication forms can be found under the "Safeguarding" section at both the nursery and child levels.

Create a medication form:

  • Nursery Level: More > Safeguarding > Medication Forms > + Create.

  • Child Level: Child > About > More > Safeguarding > Medication Forms > + Create.

There are three types of medication forms:

  • Long-Term: Medication that is administered for over 30 days.

  • Short-Term: Medication duration that is under 30 days.

  • Emergency dose: A child that has permission for an emergency dose such as Calpol to be administered.

    • If a child does not have permission, a warning will appear to prompt written permission. However, parent contact details will be shown, allowing you to confirm via telephone.


Creating a Medication Form

When creating a Medication Form you will need to add the following mandatory information:

  • Dates: Start, End, and Review (if over 30 days).

  • Medication: Name, strength, and form.

  • Reason for medication

  • Method of administration

  • Dosage: Amount, unit, and frequency (daily or as needed)

  • Timing: Before or after eating

  • Expiry date

  • Signatures: Staff receiving medication & Parent/Guardian (name & signature)

Once all information is added, click Save. This will then appear on the medication forms list.


Reviewing Medication Forms

Once added, a medication form can be reviewed anytime. You can mark its end and return date, requiring staff and Parent/Guardian signatures.

A medication form cannot be deleted or edited.


Medication Reminders

When a medication is due to expire, a reminder is sent to the nursery staff and parent 2 weeks before the expiry date and the day the medication expires. After the last reminder, the medication form is no longer available for administration.
