Accident & Incident form updates

Creating an accident or incident form on Blossom.

  • More > Safeguarding > Accidents and Incidents > Create.

Top tip: An accident form represents one child & an incident form represents two children.

Once you have selected a child (Accident) or children (Incident). The following details will show and be required.

  • The child's date of birth, room, medication information, and any allergy information.

  • Details of the incident or accident.

  • Injury details.

  • Body map & the option to add additional media.

  • Treatment details.

  • The Guardian/Contact who has been notified.

  • Date & time of the accident or incident.

  • Location.

  • Staff supervising.

When you have input all the information, you will either be able to click save & approve, or you may need to send the form for approval. This will depend on your permission level. Alternatively, if you have not completed the form you can select save as draft.

Collecting signatures

You can collect signatures in two different ways.

Paper Signatures

If you are collecting signatures on paper, select the toggle to specify when the staff member or parent has signed.

Digital signatures

Click Add Signature next to each staff member or parent. They can use their finger or stylus to sign onto the tablet.

Once the signatures are collected, the accident or incident form will be shared with the parent app.

Accident & Incident copies

If the parent requires a paper copy of the accident or incident form, the nursery will be able to export and print one from the nursery platform.
